Terms and conditions for Harmony Academy: 12 Group Classes per Month

PAYMENT & REFUND POLICY:

Payment: Before making a reservation for Harmony School, Harmony Academy Group Lessons or Private Lessons, you must attend an orientation or speak with a staff member. Full payment for all training programs is required in order to reserve any training dates. Visa, MasterCard, American Express or Discover Card are accepted and will be kept on file.

Harmony Academy Group Lessons Payment: Harmony Academy is a membership program. Payment is on a recurring monthly billing. Plan lessons are valid for one month and do not roll over to the following month.

PROGRAM POLICIES:

Cancellations and policies listed by program:

Harmony Academy Group Lessons:

  • You may cancel out of group lessons free of charge 14 hours prior to beginning of lesson.
  • Cancelations less than 14 hours of lesson will result in a $20 late cancelation fee
  • Cancelation less than 6 hours of less will be considered a “no show” and will result in a lesson deducted from your monthly plan or pass.
  • Plan Cancelation must occur before the next billing cycle, and is detailed bellow in the Reccurring Credit Card Payment Authorization section.

Harmony School:

  • First time Harmony School students are required to attend 3 consecutive weeks (15 days). Payment must be paid in full and there are no refunds or credits for missed days (Cancelation cannot be made in the initial 3 weeks of Harmony School)
  • After initial School attendance, students may schedule attendance at will.
    • Cancelation can be made 48 hours prior to scheduled school attendance free of charge.
    • Late cancelation fee of $50 will be charged for cancelations less than 48 hours prior to scheduled attendance.
    • Cancelations less than 24 hours of scheduled attendance will result in a session deducted from you plan or pass.

Private Lessons and Behavior Consultations, in home and in center:

  • Cancelation can be made free of charge 48 hours prior to scheduled lesson
  • Late cancelation fee of $50 will be charged for cancelations less than 48 hours prior to scheduled lesson
  • Cancelation less than 24 hours of scheduled lesson will result in a session deducted from your plan or pass.
  • Interruption:In the event that a training week is interrupted by inclement weather or other unforeseen event, supplementary training dates will be added as agreed upon by client and trainer. A credit maybe placed on your or a class added to your plan or pass.
  • Reschedule: Session may be rescheduled at any time according to the cancelation and attendance policies listed above.

Plan and Pass Cancellations/Refund:

  • You may cancel your plan or pass at any time. Unused lessons will remain on your account until their expiration date. Full or partial refunds for unused lessons will not be offered unless your cancellation was suggested or approved by a staff member.

Recurring Credit Card Payment Authorization

You authorize regularly scheduled charges to your credit card. You will be charged the amount indicated on your selected plan each billing period. A receipt for each payment will be provided to you and the charge will appear on your credit card statement. You agree that no prior-notification will be provided unless the date or amount changes, in which case you will receive notice from us at least 10 days prior to the payment being collected.

I understand that this authorization will remain in effect until I cancel it in writing, and I agree to notify DIH in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. If the above noted payment dates fall on a weekend or holiday, I understand that the payments may be executed on the next business day. I acknowledge that the origination of Credit Card transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this Credit Card and will not dispute these scheduled transactions; so long as the transactions correspond to the terms indicated in this authorization form.

AUTHORIZATION I hereby authorize DIH to charge the indicated credit card for fees associated with services provided, including, if necessary, adjustments for any changes to my account. I agree that the periodic charge will be applied to my credit card according to my DIH account billing cycle, and in order to cancel the recurring billing process, I am required to contact DIH 15 days in advance to either cancel the associated DIH account, or arrange for an alternative method of payment. I understand that DIH will not provide to me any invoices or bills. I agree that if I have any problems or questions regarding my account or any services provided by DIH, I will contact DIH for assistance using the contact information on their website at www.HarmonyDogs.net. I also agree that I will not dispute any charges with my credit card company without first making a good faith effort to remedy the situation directly with DIH. I guarantee and warrant that I am the legal cardholder for this credit card and that I am legally authorized to enter into this recurring credit card billing agreement with DIH.
